Mr. Chad Wayne Collinsworth, 35, of Lenox, died  suddenly, April 27, 2012 from injuries sustained in a automobile accident. He was a native of Pensacola, Fla., and a former resident of Pace, Fla., before moving to Alabama in 1991. He was of the Baptist faith, a 1995 Graduate of T.R. Miller High School, and was employed at T.R. Miller Mill Company as a stacker machine operator.
He is survived by his mother and stepfather, Charlene and Terry Jackson of Lenox; his father, Wayne Collinsworth of Miss.; a son, Eli Collinsworth of Lenox; a daughter, Faith Collinsworth of Lenox; a brother, Michael Collinsworth of Andalusia; his paternal grandfather, Leroy Jackson of Pensacola; and his fiancee, Mischelle Price of Lenox.
Funeral services were held Monday, April 30, at 2 pm. at Williams Memorial Chapel Funeral Home with the Rev. Mike Skipper officiating. Buriall followed at Damascus Cemetery, with Williams Memorial Chapel Funeral Home directing.
